Analysis of Intrahepatic Cholangiocarcinoma

Abstract
Intrahepatic cholangiocarcinoma(IHCC) is malignant tumor arising intrahepatic duct of the liver and difficult to differentiate from hepatocellular carcinoma(HCC). Because of poor prognosis, acurate diagnosis of IHCC is important for the proper treatment. Author analyzed retrospectively ultrasound and computed tomography(CT) findings of the IHCC and tried to find characteristic findings of IHCC.
Ultrasound (n=32) and computed tomographic (n=36) findings in 37 patients with intrahepatic cholangiocarcinoma (IHCC), confirmed by operation (n=8) and percutaneous biopsy (n=29), were reviewed to detect characteristics of IHCC. Male to female ratio was 24:13 and age ranged from 32 to 72 years (mean:57 years).
Us was performed in 32 cases of IHCC. The majority of IHCC was isoechoic or hyperechoic than the liver parenchymal echogenesity and all four cases of the lesions larger than 10cm in diameter were high echogenic. Compared with normal parenchymal density, the border of the tumor was ill defined in 19 cases, relatively well defined in 4 cases and sharply defined in 9 cases. The margin of the mass was irregular in 18 cases and smooth in 14 cases. In 9 cases, peripheral halo was demonstrated. CT findings in 36 cases of IHCC were showed ill defined mass in 14 cases and well marginated mass in 8 cases. The margin of the mass was irregular in 32 cases. Low density mass was in 34 cases and inhomogeneous mass was in 22 cases. Intrahepatic ducts were dilated in 31 cases and calcification within the mass was seen in 10 cases. Also exrahepatic extension such as lymph node enlargement was demonstrated in 24 cases (66.7%), mainly in hepatoduocenal ligament(n=16), portocaval(n=15), intercavoaorti node(n=10). Direct invasion to omentum, gallbladder and stocach was noted in some cases, and portal vein or IVC thrombosis was associated. The purpose of this study was analysis of above US and CT findings of IHCC and made accurate diagnosis, can helpful for proper treatment and prognosis of IHCC.
